AST (Advanced Surface Treatment) offers many improvements over other refractive surgery procedures. These include a safer more predictable outcome with no more lasik flaps. Before a surgical laser can be used to correct your vision in LASIK eye surgery, a flap must be cut in the clear covering over the front of your eye (the cornea) and folded back to give the laser access to the tissue beneath. The flap is cut using either a blade, called a microkeratome, or a laser, such as Intralase. Once the flap has been cut and folded back, the excimer laser then reshapes the front of your eye to correct myopia, hyperopia, and astigmatism. After LASIK eye surgery, the corneal flap is replaced and allowed to heal on its own. It is during this replacement and healing process, or during the actual cutting of the flap, that most complications occur. An imperfect or incomplete cut, or irregular healing can make your outcome less than expected. Most patients require no corrective eyewear after surgery, although patients over 45 often need reading glasses for an unrelated condition called presbyopia.

AST & Custom AST - Am I a candidate?
Not everyone is a good candidate for Laser Vision Correction. When you come in for an evaluation, your eyes will be examined to determine whether LASIK, AST or another refractive procedure is appropriate for you and whether you are at risk for any complications.
The ideal Laser Vision Correction candidate:
- Is over 18 years old;
- Has had stable vision for at least six months;
- Has a healthy cornea;
- Has refractive error(s) that fall within the treatable range;
- Does not have a disease or condition that could impair the procedure or healing process;
- Has been educated about the procedure including its risks and benefits.
- Understands that the goal of surgery is to improve vision and reduce dependence on glasses and contact lenses.
As with traditional Laser Vision Correction, not everyone is a candidate for Custom. Sometimes patients have to wait until the particular correction they need is approved by the FDA. If you are not a candidate now, you might decide to undergo Laser Vision Correction without the CustomCornea feature. Or you might choose to wear glasses or contact lenses and wait until the technology is available to treat your specific condition.

The Procedure

On the day of your surgery, you will come into the office and change into a gown and cap. You'll lie down on your back on the operating table. The laser will be above your head. You'll be given anesthetic eye drops so you will have minimal discomfort, mainly a slight sensation of pressure during the procedure. Dr. Mang will talk to you throughout the surgery so you know what is happening.
If you are undergoing custom Laser Vision Correction, a disk with your eye information from the CustomCornea Wavefront exam is loaded into the LadarVision machine. Dr. Mang uses this information to fully customize your Laser Vision Correction treatment, using the data to guide the laser to correct your corneas' individual irregularities.
At NVLC we use the LADARVision® 4000 for laser vision correction. Its fine, 0.8mm laserbeam is capable of moving all around the eye and doing a custom procedure so patients' astigmatisms can be corrrected to the minutest level possible with today's technology.
Not all excimer laser systems are created equal. The beams of light they produce can vary considerably, both in their shape, size, and nature. The LADARVision® system uses a flying small-spot laser beam that's just 0.8mm wide. So it is capable of removing your corneal tissue in very small or "subtle" increments, in both traditional and CustomCornea® treatments.

Recovery
After the Laser Vision Correction procedure, you'll step out into the recovery room and rest for about ten minutes. Dr. Mang will then come out to examine you. If everything looks good, you'll be able to go home with your designated driver. You'll come back the next day for a follow-up appointment, after which follow-up appointments will be scheduled for two months, six months and one year.
If you had the AST procedure a clear contact lens was placed on your eye at the end of the procedure to protect the eye. Dr. Mang will remove it at your follow up appointment. With the LASIK procedure, the eye heals on its own within a few days with no need for stitches. A common complaint after surgery is sensitivity to light, but this will subside. Antibiotic eye drops will be prescribed for a few days, along with any other post-operative instructions. Full recovery takes a few weeks.
The results of laser vision correction last for a lifetime. However, most patients will eventually need reading glasses as they age and develop presbyopia, a vision condition brought on by age that makes nearby objects seem out-of-focus.
